Free community support services to promote successful discharges from either in-patient hospital stays or emergency departments * Provides individuals to access services for a 4-6 week period to regain strength and independence.
Services may include:
- frozen meal delivery
- transportation home from hospital
- transportation for 2 follow up medical appointments
- homemaking services
- home safety assessment
- wellness check ins
- communication with other support services
- access to a 24/7 support line

Older adults with function impairment being discharged from hospital. Must be medically stable and able to transfer independently.
