Description des services:	No charge community support services to promote successful discharges from either in-patient hospital stays or emergency departments * provides individuals to access services for a 6 week period to regain strength and independence * services include: hot or frozen meal delivery transportation for 3 follow up medical appointments homemaking services - 3 visits within 6 weeks home fall safety assessment weekly wellness check ins for isolated referrals service navigation respite (for patients with cognitive decline/diagnosed dementia)
